In the Pilots Handbook for the 109E that i have are only the two lower handles mentioned.
There are also only 2 handles in the pictures.
The upper handle ,middle in your pic and cod, is to change the ignition to early firing to start the engine and/or to clean the sparkplugs.
The lower handle is for the clutch between the inertia-starter and the engine.
The upper handle in cod seems to be for cutting off fuel and ignition in case of emergency.
The handbook says after winding up the inertia starter give 2 to 4 pushes to the priming pump, set magnetos to 1+2 and pull first the ignition handle and then the clutch handle.
Release handles after the engine has started.
