Visual effect settings med/high

So far, I am happy with the patch..and I think I have fps boosts in both high and med visual effect settings. My question is, WHAT if any are the changes that enabling medium visual effects from high actually do in game? Personally, I can't tell any appreciable difference (which is good) but I'm sure there are some and some players do notice them. Curious, thanks.
