The fact is that the new HD link does only appear if the video was originaly rendered at HD Widescreen resolution, and if the viewer has enough bandwith. If one or neither of these two conditions are not acomplished, the only link that appears is the old "Watch in High quality", that is the 640x480 you are referring at.
the new HD view is far superior than that "High" quality, and I can assure that in this mode, the youtube video barely lose any quality from the original file I have on my Pc
