Welcome to IL2COD bug # 890,001! This is a known issue that hopefully is caused by some sort of graphical bug. But the easiest way to avoid them is knowing how to know they're ghosts long before you get there. The easiest way to do this is to fly straight at them (especially if you see a formation of them). If you don't see the dots changing altitude (as you'd see a group of humans in formations) but instead they are stuck in one spot.

The other way is to pay attention to the radar contacts. They never call out the ghosts.

dumb question sort of related - when playing on multiplayer servers with full realism on like the ATAG server, is there way of telling where I am on on the map since I can't see the icon of my plane on the map? Is it a case of looking around and trying to spot landmarks or are there any better methods for finding / working out my location?

Only way is to open the map (you can zoom all the way in on it) and use it to spot landmarks and where you are. It doesn't take long to get navigation down though. There is a learning curve, just like anything, but after a while you'll wonder why you ever flew with icons on, at least IMO.
