Previously I seem to have gained access to Western and Eastern Islands almost immediately after I came to Greenwort. Now I have played long after that but I can't get the map there. Who is supposed to give it?

Fisherman in Greenwort, I believe. Go through the dialogue options enough and you'll find it.
