palker4 is right.

1. Steam reports the top 100 games sorted by players playing the game.
2. Players count in both, single AND multiplayer - as long as they are connected to steam.
3. Players using the Steam "Offline Mode" won't count
4. Games with less players than position 100 will show up with a 0 since steam doesn't output them on their stat page.
