Have you tried
VAC (Voice Activated Commands)?
It is not a freeware, but it's cheap ($ 18 ), and the trial period is for 14 days.
With VAC it's easy to create and edit profiles for all sort of games, and it works very well in IL2: I'm using it since several years ago, and it helps me a lot to command my AI wingmen avoiding to use the console.
