I just turn it to "On" and leave it off in-game. I've heard the adaptive V-sync gives it more range when the fps drops (which means it doesn't necessarily reduce to half your refresh rate), but as quick as it happens, it doesn't make much difference on my machine. (I think split seconds of 45 fps vs 30 fps isn't much when they are occurring at micro-second intervals.)
