I don't know how good/realistic they are (it's also a matter of taste) and if it depends on hardware, but the in-cockpit sounds are definitely useful.

I fly 109s and 110s with CEM and temp effects enabled, which requires constant adjustment of the prop pitch. Well, thanks to the sounds i can tune my engines properly and keep within the operating limits without looking at the instruments too much.

I just set it to max continuous power with appropriate radiator positions and listen. Then, during combat/climbs/dives/etc i adjust prop pitch by ear, trying to maintain the same sound, which enables me to look at the bandits instead of my tachometer.

In short, they might not be perfect but they are dynamic and very useful for listening to the engine "talk" to you.
