Nub question...but how do you do this ingame?

You can ugrade the item in the "HERO" meny by clicking it with right mouse button and chosing "upgrade", then you will have a fight with the keepers of the item. If you win, the item will be upgraded.
Also remember, that quite few items can be upgraded.

while on the subject; do all upgradable items have the greyed out text in the infobox or are there some (maybe with only 1 upgrade?) that don't show this text?
